Submission Requirements

• Submissions received within 15 to 90 calendar days prior to the effective date.
• ACORD forms or competitor applications
• Vehicle list* – year, make, model, VIN, and stated amount.
• Driver list* – name, DOB, state licensed, DLN, and date of hire
• IFTAs – 4 full quarters including most recent
• Loss Runs – current year + 4 full years (valued within 120 days)
  * Excel format preferred
• For all submissions with 15-24 power units, a completed Exposure Template is required for rating.
